Pet Health Requirements

Before attending classes at Waggles, the following health information must be verified:

  • Date last vaccinated for:  parvo/distemper/rabies

  • Date of last fecal test and result of same (must be within two months)

  • Name of flea/tick preventative and date when last administered

Must see date of last vaccination—not a reminder of when it is next due. Please check your records carefully.

Payment

Payments can be made by cash, check, or online once class placement has been confirmed.

RELEASE & HOLD HARMLESS STATEMENT

I understand and assume the possible risks inherent to all dog-related activities, including, but not limited to, bodily injury to myself, my dog, or to anyone accompanying me to the training classes at any location such training classes are offered, including, but not limited to, the premises located at 91 Merchants Lane, Williston, Vermont.

I further understand that this relate and hold Harmless Agreement limitation and will remain in full force and effect as long as I train with Waggles, LLC, regardless of the location of such training classes and regardless of what dog I am training.

Therefore, in consideration of the privilege of training my dog with Waggles, LLC, I do herby agree to hold harmless and indemnify Lori Bielawa, individually and as a member of Waggles, LLC, and anyone that might assist her with classes or instruct in her absence, and also the owners of any locations where such training classes are offered, of any from any claim for damages and/or injuries I might sustain while attending training classes, and further release her from any liability or responsibility for any accident, damage, injury, or illness to me, may be.

I understand that the training premises as used in the Release and Hold shall include any location where such training classes are held, including, but not limited to the premises located at 91 Merchants Lane, Williston, Vermont.
